The Partner plan is designed to compliment long-term care insurance policies. At the time of enrollment, you must be living independently and not require in-home health support services. If you have a long-term care insurance policy or have funds set aside for a long-term care facility, this is the perfect way to bridge the gap between now and when you might transition into an assisted living or nursing facility. 

The partner membership includes:

• all of the services that the Comprehensive membership offers with one exception, there is no discount on a long-term care facility. 

With this membership, Care At Home will coordinate and provide for your medically necessary in-home services and technology needs.  However, once a member is determined to need the support of a long-term care facility, our in home services will cease. The Care At Home team will help you make the transition from your home to your chosen long-term care facility.

The Partner membership requires:

• a health and lifestyle assessment,
• one time enrollment fee and
• monthly maintenance fees. 

Fees are based on the health and age of the individual at time of application.
