The Coordinator plan is designed for individuals who need services in their home immediately. Unlike the Comprehensive and Partner plans, there is no requirement that you are living independently and not receiving in home support services.
The Coordinator membership includes:
- all of the services that the Partner membership offers with one exception, you pay for the cost of services at an hourly rate as you need and request them.
With this membership, Care At Home will coordinate and provide for your medically necessary in-home services and technology needs. However, once a member is determined to need the support of a long-term care facility, our in-home services will cease. The Care At Home team will help you make the transition from your home to your chosen long term care facility.
The Coordinator membership requires:
- a lifestyle assessment,
- annual enrollment fee and
- monthly maintenance fees
The Coordinator membership does not require a health assessment to gain membership. The fees vary per individual based on the amount of services needed per month. All services are charged at an hourly rate.
