Should health problems arise and complicate your life, your personal care coordinator will be your advocate and resource to identify and facilitate the support you need.
The cost of membership is affordable and predictable and with the support of your Personal Care Coordinator, Home Technology and other services, you can continue to live in your own home. Care At Home offers you three separate membership plans to meet your specific needs. These plans include the Comprehensive, Partner and Coordinator.
