Basic Life Insurance/Accidental Death & Dismemberment

Description

Full time employees receive 1.5 times your annual salary for both life insurance and accidental death and dismemberment. Part time employees receive a flat $10,000 benefit. Paid by Mercy Regional Health Center.

Optional Life Insurance for Children

Description

$2,000, $5,000, $10,000 or $15,000 coverage on eligible dependents.

Description

Allows you to buy from $10,000 to $800,000 of life insurance at competitive group rates.

Provided Short-Term Disability Insurance

Description

After 14 days, provides disability payments for up to 90 days. Employee is eligible to receive up to 60% of base pay.

Optional Long-Term Disability Insurance

Description

After 90 days of disability, you may choose to receive up to 60% of your monthly salary, not to exceed $5,000 per month. Amount is offset by other income, such as social security.
