Healthcare Spending Account

Description

Allows you to pay out-of-pocket healthcare expenses that are not paid by medical/dental insurance plans with pre-tax dollars.

Dependent Care Spending Account

Description

Allows you to pay for childcare and dependent-adult care costs with pre-tax dollars.

Basic Life Insurance/Accidental Death & dismemberment

Description

One times your annual salary for both life insurance and accidental death and dismemberment.  This benefit is paid by the employer

Optional Life Insurance for Children

Description

$2,000, $5,000, $10,000 or $15,000 coverage on eligible dependents.

Optional Life Insurance for Employee/Spouse

Description

Allows you to buy from $10,000 t $800,000 (may purchase up to 5 times your base annual salary) of life insurance at competitive group rates.

Long-Term Disability Insurance

Description

After 90 days of disability, you may receive up to 60% of your monthly salary, not exceed $5,000 per month.  Amount is offset by other income, such as social security.

Short-Tem Disability Insurance

Description

After 14 days of being away from work due to an accident or an unexpected medical condition, benefit will pay 60% of your basic monthly earnings to a maximum of $5,000 per month.

Automatic coverage after one year of continuous full-time employment
