Via Christi Healthcare Outreach Program for Elders, Inc. (Via Christi HOPE) opened in September of 2002 as the PACE (Program of All-Inclusive Care for the Elderly) organization for the Via Christi Health System. Via Christi HOPE was the first PACE site in the State of Kansas.
Via Christi HOPE assists older adults to remain safe and and independent in the community for as long as possible. This is accomplished through high quality, health and social services tailored to meet the unique needs of each individual.
Via Christi HOPE services include, but are not limited to:
- Activities
- Home Care
- Medications and Medication Management
- Primary Medical Care
- Wheelchair Accessible Transportation
- Physical, Occupational and Speech Therapies
- Social Services
- Nutrition Counseling
- Personal Care
